Testing templates 1226

I am new to this community and also to SOX as well.
Also, I am the only accounting personnel with public accounting background for my employer.
In the past 6 months, I’ve been working under directions of this consultant, who claims to be a SOX expert. He created a template for test of design effectiveness and another one for testing the operational effectiveness. The problem is these templates were not designed user-friendly. Too much tidious work to fill out these formats.
Can anybody share how to document the testing procedures and conclusions for test of design effectiveness and test of operational effectiveness?
